Yes. We produce the layouts and hand them to your developer. In that case we document sizes, spacing and behaviour in extra detail so the build doesn't drift from the design.

Usually yes. Finding and fixing the three to five points that hurt conversion most is cheaper and faster than a full redesign. We start with an audit.

There's an approval at each stage — direction, wireframe, visual. It isn't a revision counter but stage-by-stage sign-off: reopening an approved stage counts as extra work and is quoted in advance.

It depends on scale: on smaller projects analytics and a heuristic review are enough, while for larger products we propose a short test with real users.
